Northeast Kingdom Human Services (NKHS) is seeking an Embedded Mental Health Specialist to provide immediate and time-limited response to clients in Emergency Departments, aiming to reduce barriers to community resources and treatment services.
Requirements
- Ability to perform complex communication, ability to engage individuals to insure their quick access to community supports.
- A broad understanding of local services and the ability to connect clients to these services to meet their needs.
- Ability to effectively interface with a wide range of medical and professional clinicians in hospital/clinic settings; & with partner community agencies.
Responsibilities
- Provide resource information and immediate access to services.
- Early case coordination, next day follow-up and triage appointments.
- Convey hope, confidence, professionalism, offering peer contact info.
- Refer for any follow-up crisis screening (at or away from Emergency Department), refer for any identified mental health (MH) or substance use (Addiction) assessment and treatment needs, refer to NKHS Out-patient case manager for longer term coordination with MH and Addiction services.
Other
- Minimum of a Bachelor’s degree in a recognized field of human service.
- Experience managing and supporting client’s needs
- Experience in provision of public or private human service.
