Western Wayne Family Health Centers is looking to support the development, implementation, and monitoring of the Quality Improvement (QI) program to ensure compliance with regulatory standards and promote a culture of continuous quality improvement.

Requirements

  • Strong Data Analytics with Excel, Azara, Athena.
  • Data Hygiene strongly preferred
  • Knowledge of computer, systems, and applications including data analytics.
  • Experience with EHR systems
  • Experience with Azara DRVS
  • Experience with data visualization tools
  • Experience with reporting platforms

Responsibilities

  • Extract and analyze data from Azara DRVS, EHR systems, and other tools to support internal performance tracking and external reporting requirements.
  • Support the development of dashboards and data visualizations for leadership, care teams, and external partners using Azara and other reporting platforms.
  • Conduct regular audits and chart reviews to assess clinical documentation accuracy, data integrity, and compliance with HRSA, UDS, and PCMH standards.
  • Collaborate with IT department to optimize EHR templates, clinical decision support tools, and reporting functionalities that support quality improvement efforts.
  • Validate and maintain accurate data for Promoting Interoperability measures, including clinical quality reporting, electronic exchange of health information, and patient engagement indicators.
  • Facilitate data submissions to external agencies such as UDS, NCQA, and state health departments, ensuring timeliness and accuracy.
  • Support population health initiatives by identifying at-risk patients, monitoring care gaps, and tracking outcomes related to chronic disease management and preventive care.

Other

  • Bachelor’s degree in health sciences/ business or bachelor’s degree with five (5) years’ experience in relevant fields.
  • Minimum of two years of administrative experience, including one year experience in a health care organization.
  • Ability to maintain composure under stressful conditions
  • Ability to maintain accurate records and prepare reports and correspondence related to the work
  • Skilled in developing and maintaining effective relationships with medical and administrative staff and with patients and the public.
