The Individual Skill Developer (ISD) provides direct, person-centered, and recovery-focused services to adults living with mental health and/or substance use challenges. The ISD supports members in building practical skills that promote independence, housing stability, and overall quality of life.
Requirements
- Ability to build trusting, professional relationships with members from diverse backgrounds.
- Strong interpersonal and communication skills, with the ability to model and teach practical life skills.
- Problem-solving ability, flexibility, and creativity in supporting members’ individualized needs.
- Valid driver’s license, reliable transportation, and ability to transport members when necessary.
- Must meet all state and agency background check and training requirements for CCS providers.

- Pass a criminal background check and be free of offenses prohibiting employment under HSF12 at the time of employment, and annually thereafter.
Responsibilities
- Provide individualized skill development and enhancement services, including: Communication, interpersonal effectiveness, problem-solving, decision-making, self-regulation, and conflict resolution.
- Daily living skills such as personal care, household management, financial literacy, shopping, cooking, and safe use of transportation.
- Accessing and navigating community resources, including healthcare, employment, education, and social supports.
- Deliver services through methods such as modeling, mentoring, monitoring, cueing, and hands-on assistance in real-life settings.
- Provide services one-on-one and in small groups to promote social connection and peer support.
- Support members in developing skills that enhance housing stability, community integration, and self-sufficiency.
- Maintain accurate, strengths-based, and timely documentation of all services provided in compliance with CCS, Medicaid, and agency standards.
Other
- Bachelor’s degree required; master’s degree preferred.
- Degree in human services, social work, psychology, or a related field preferred; other fields may be considered with relevant experience.
- Experience working with adults who have mental health and/or substance use challenges preferred.
- This position requires meeting members in a variety of settings, including their homes, community spaces, and program facilities.
- Some flexibility in scheduling is required to accommodate member needs.
