Requirements
- 3+ years of post-residency work experience
- State licenses in WI, MN, IA
- Must be currently enrolled, or eligible for enrollment, as a Medicare provider
- Active IMLC
- Board-Certified Family Physician
- Primary care experience and behavioral health experience, ability to treat both acute and chronic care conditions
- Should be tech-savvy, proactive, organized, detail-oriented, and have telemedicine experience.
- Must have prior experience with EPIC EMR
Responsibilities
- Provide on-demand Acute Care and scheduled Primary Care
- Manage patient follow ups for chronic care programs
- Co-manage patients with external partners within our healthcare delivery model
- Prescribe medication and refills as appropriate
- Drive high-quality care with a focus on patient outcomes
- Elevate remote care by providing compassionate and meaningful patient care
- Collaborate with Clinical Team Members to build clinical guidelines and policies
Other
- Must be a board-certified Family Medicine physician
- Clean background and medical malpractice history
- Extensive experience handling 3+ cases per hour
- Willing to commit 40 hours per week, 36 clinical hours, and 4 administrative hours
- Must be able to work one weekend day every other week
