Coarc is seeking a compassionate and organized Residential Plan Developer to support individuals with intellectual and developmental disabilities by creating meaningful plans that reflect their goals, dreams, and personal choices.
Requirements
- Exceptional written, communication, and computer skills
- A valid driver's license.
- Ability to be certified in all required trainings. (Med Adm/Insulin Adm, CPR/FA, PROMOTE/SCIP)
Responsibilities
- Advocate for and support individuals in making their own life choices and achieving personal goals.
- Develop and implement individualized Staff Action Plans (SAPs) aligned with each person's Life Plan.
- Participate in Life Plan meetings and collaborate with care managers and service providers.
- Promote self-advocacy, independence, and community inclusion.
- Maintain accurate documentation and communication with all stakeholders.
- Build trusting, respectful relationships with individuals and their Circle of Support.
- Provide hands-on support as needed, in accordance with the service plan.

- Associate's degree preferred
- 1-2 years related experience and training required
- Occasionally required to lift up to 50 pounds.
- Loves person-centered work and enjoy building supportive relationships.
- Is empathetic, patient, and positive, and naturally supports others in making their own choices.
